community care team
This small team works with people with dementia in their own homes and out and about in the local community. We offer a borough wide service from Monday to Friday and our office hours are from 9.00am to 5pm.
We enable people to remain independent and help people to use community resources. We encourage self-care skills, and this includes helping people to shop, prepare food and maintain their personal hygiene. We introduce people to day centres, residential care and lunch clubs, ensuring that the person adjusts well to their new surroundings.
We can also help people to manage their own finances such as paying bills, cashing pensions and going to the bank
We aim to create an environment that helps the person with dementia to live with dignity and self-respect
A named member of staff is responsible for each person we provide the service for.
carers dementia support services
The Carers Dementia Support Service provides home-based respite to carers of people with Alzheimer's and other forms of dementia. We operate throughout the Borough of Westminster, providing up to 12 hours of respite per week in the person's own home, and can provide care throughout the night.
With our team of dedicated relief carers, we provide consistency and continuity. We aim to support carers and enable them to continue caring for their friend or relative within their own home for as long as possible.
This flexible service is responsive to the changing needs of the individual. We can provide stimulating and therapeutic activities by going on outings to galleries, cafes and parks, or one-to-one activities within the home.
home care plus
Home Care Plus is home care with a difference. We work specifically with people with dementia, and offer intensive, person-centred support. We provide practical support and personal care, and promote emotional well-being. HCP can also provide continuity while the person is in hospital or respite care, by continuing our regular visits. Visits can be up to 3 hours a day, 4 times a day, and is open seven days a week from 7am to 9pm.
We aim to build trusting relationships with people in order to provide effective care.
We engage in meaningful activities with people at home or out and about, and link people to services, family and friends, through local visits and escorting.
tresham day centre
The day centre is open seven days a week from 8.30am to 5pm and provides a service for older people with dementia living in north Westminster. The centre is small taking a maximum of fifteen people a day, and has as its ethos an aim to encourage each service user to be as independent as possible.
Each day a choice of activities takes place that includes reminiscence, art, discussion, relaxation, tea dances and much more.
The centre is well supplied with audio/visual and sensory equipment, which enables those who are less mentally able to participate more fully.
We also provide meals and can cater for individual tastes as well as religious and cultural needs.
The day centre has its own transport which means greater flexibility in our service provision, and has a pleasant garden in which people can relax or take part in various activities.
