
Community Support
Community support focuses on enabling or re-enabling people to do as much for themselves as possible. Our services aim to create an environment which helps a person with dementia to live with dignity and self-respect.
Westminster
Our community care team in Westminster:
- work with people with dementia in their own homes and out in the local community
- help people to use community resources
- encourage service users' self-care skills, such as shopping, preparing food and maintaining personal hygiene
- introduce service users to day centres, residential care and lunch clubs, ensuring that they adjust to their new surroundings
- help people to manage their own finances, such as paying bills, and going to the bank.
Hammersmith and Fulham
Our pioneering dementia and Korsakoff's social inclusion and activities service, focuses on accessing community facilities and resources.
The service aims to support individuals with low to moderate dementia or Korsakoff's to remain involved in or gain access to, activities of their choice.
These activities include:
- leisure activities
- education
- volunteering
- physical exercise
- services targeting older people, such as luncheon clubs
- access to therapeutic interventions - group cognitive stimulation therapy or creative therapies.
The service is available daily, including evenings and weekends, and keeps people involved in social and individual networks enabling them to feel included, care for each other and create long-lasting relationships.
