Health and care matters

Housing 21 is experienced in providing care and support services but we also work with partner agencies using our buildings to accommodate a range of health and care services for older people. Our extra care housing can act as a centre for local health services meeting PCT priorities and helping the Government drive for care out of hospitals.

Housing 21 are committed to ensuring that our services enable residents to retain maximum independence and choice at all times. We work with Primary Care Trusts (PCTs) and Social Services to support older people in the community and to enable people to move back home quickly and with appropriate support after a spell in hospital. This not only offers older people better services but also supports local councils and health communities to meet Government targets.

In many of our extra care housing schemes we offer rehabilitative and respite care by using some of the flats for short-term support. For example in Bristol, Housing 21 in partnership with the PCT and social services offer intermediate care in four flats in the scheme. By providing a housing based model of rehabilitative support we are able to assist older people to test their competence and confidence and return to living independently at home.

In our communal and community spaces we offer treatment rooms for health care staff to support people in the scheme and in the surrounding community. This includes chiropodists, district nurses and health visitors, podiatrists, memory clinics and other vital services which bring health care close to the home.
